Managing Chronic Medical Conditions at School

Please provide completed care plan if your student has any of the chronic conditions listed below.   Please note that any medications that need to be taken during the school day will require a Medication Authorization form completed and signed by parent and the student's physician .

Return care plan and medication authorization to your school health office.

Help us care for your child in the best possible manner by filling out the forms completely with input from your child's physician.
